Metabolic Acidosis in Acetaminophen Overdose without Concurrent Liver Toxicity

Introduction Metabolic acidosis and coma are common complications of acetaminophen overdose. These affects are usually attributed to massive hepatic necrosis caused by Nacetyl-p-benzoquinoneimine (NAPQI), a toxic metabolite of acetaminophen. We describe an unusual case of lactic acidosis and coma in a patient with acetaminophen toxicity without associated hepatocellular damage as the identifiable cause.
